Prof Clara Chow

Prof Clara Chow is Academic Director of the Westmead Applied Research Centre, Faculty of Medicine and Health, University of Sydney. She is a cardiologist and Clinical Lead of Community Based Cardiac Services at Westmead Hospital, and a member of the Western Sydney Local Health District Governing Board. Prof Chow is also the Director of the Australian Stroke and Heart Accelerator. She was honoured with a Telstra Brilliant Woman in Digital Health Award in 2022 and is a 2023 Honour of Australia recipient. She currently holds an honorary appointment as the Charles Perkins Centre Westmead Academic Co-director and is the immediate past-President of the Cardiac Society of Australia and New Zealand.
Prof Chow’s research focuses on the prevention of cardiovascular disease, innovation in the delivery of cardiovascular care and the evaluation of digital health interventions. She has expertise in the design, delivery and implementation of clinical trials. Her PhD from the University of Sydney, Australia was in cardiovascular epidemiology and international public health and her post-doctorate from McMaster University, Canada in clinical trials and cardiac imaging. She has over 250 publications including papers in internationally leading medical journals NEJM, JAMA and Lancet, and she is supported by a NHMRC Investigator grant.

Dr Aashish Contractor

Dr Aashish Contractor is currently the Director of Rehabilitation and Sports Medicine at the Sir H.N. Reliance Foundation Hospital, in Mumbai, India, and a fellow of the American College of Sports Medicine. The department provides rehabilitation services for all medical specialties and works with elite athletes across the country. He is widely recognised as the pioneer of cardiac rehabilitation and sports cardiology in India, and has authored The Heart Truth, a book on heart disease prevention.
On the academic front, Dr Contractor worked and trained in the United States, under leading experts in his field. He was the founding Vice-Chair of the International Council of Cardiovascular Prevention and Rehabilitation, which includes more than 45 countries around the world. He is also the Certification Director of the American College of Sports Medicine (ACSM) in India.
Dr Contractor was the Medical Director for the Standard Chartered Mumbai Marathon from 2004 till 2014 and oversaw other international sporting events in India. He is an avid sportsman himself, having run several marathons.

Dr Chan Ngai-Yin

Dr Ngai-Yin Chan received his MBBS degree from the University of Hong Kong in 1992 and was conferred Doctor of Medicine [MD(HKU)] in 2019. He was conferred FACC, FRCP(Edin), FRCP(Glasg), FRCP(Lond) and FHRS in the years of 2005, 2006, 2010 and 2012 respectively. He is currently Chief-of-Service and Consultant Physician in the Department of Medicine & Geriatrics in Princess Margaret Hospital and North Lantau Hospital. He is also an Honorary Clinical Associate Professor of the Department of Medicine & Therapeutics of the Chinese University of Hong Kong. He has been taking very active roles in different professional organizations in Hong Kong. He was the Immediate Past President of the Hong Kong College of Cardiology.
He was the past President of the Hong Kong Interhospital Network of Pacing and Cardiac Electrophysiology. His research interest is on preventive cardiology , cardiac rehabilitation, catheter cryoablation, epidemiology and community screening of atrial fibrillation, ICD and pacing therapy and left atrial appendage occlusion,. He is the first author of over 50 peer-reviewed manuscripts and abstracts. He is the editor of the textbook titled “The Practice of Catheter Cryoablation for Arrhythmias”. Dr Chan is an all-round cardiologist and has published extensively in the areas of cardiac electrophysiology, interventional cardiology and preventive cardiology in high-impact journals like the Journal of American College of Cardiology, Heart, Europace, Heart Rhythm and Canadian Journal of Cardiology. He is the Editor-in-Chief of the Journal of Hong Kong College of Cardiology. With his outstanding performance both professionally and in the community contribution, he won the Hong Kong Ten Outstanding Young Persons Award in 2006, Hong Kong Humanity Award in 2015 and was awarded as the Outstanding Staff of Hong Kong Hospital Authority in 2022.

Dr Sa'ari Bin Mohamad Yatim

Dr Saari Mohamad Yatim graduated with MD from University of Sciences, Malaysia and obtained his Master Degree in Rehabilitation Medicine from University of Malaya, Kuala Lumpur. He then pursued his studies in Cardiopulmonary Rehabilitation at Medical Department, Tung Wah Hospital, Hong Kong and Chonbuk University Hospital, Korea.
Dr Saari sits on several committees in Malaysia and internationally. He is currently the Deputy Head of Rehabilitation Medicine services in the Ministry of Health, Malaysia and the Head of Rehabilitation Medicine Department, Serdang Hospital, Selangor. He is also an executive board member of the Asia-Oceanian Society of Physical and Rehabilitation Medicine and AsiaPRevent.

Ms Constance Chan

Constance Chan, Principal Medical Social Worker (MSW) at the Changi General Hospital has 13 years of extensive experience working with patients with renal or cardiology condition.
As a social worker, she engages patients to address concerns relating to financial, care, adjustment to illness and relationship issues. She works both at an individual level and family systems level to work through issues of grief and loss, marital issues and personal problems.
Given Constance’s strong interest in using creative avenues to engage patients of different profiles, she has attended various courses to get herself equipped. She has a Degree in Arts and Social Sciences (Social work), a Masters in Counselling and is also a certified Sand Play Practitioner and a Nagomi Pastel Art instructor. Recently, she also obtained a certificate in mindfulness facilitation. She uses her skills and knowledge to bring patients, caregivers as well as staff on a self-exploratory journey of their feelings and taps on their strengths to manage life challenges.

Dr Jocelyn Chew

Dr Jocelyn Chew started her research interest in heart failure self-care, where she discovered the importance of preventive and precision medicine. She had since been researching on the upstream problem of cardiometabolic conditions, especially in obesity prevention and management. She researches and innovates on the use of artificial intelligence (AI) to improve health behaviours, cardiometabolic disease prevention and healthcare service delivery. She has developed multiple self-regulation programmes for cardiometabolic disease management, including an AI-assisted smartphone app for personalised weight management. Dr Chew is passionate about healthcare digital transformation and has received training in nutrition science, AI and advanced statistics. She is currently a Singapore Biodesign Innovation Fellow to further her research and innovation endeavours in medtech and entrepreneurship.
Dr Chew has published 50 internationally peer-reviewed articles of high impact factor, presented her research in various conferences globally, and received multiple international awards over the years. She had been awarded close to SGD 800,000 worth of research funding and is an editor of Frontiers in Nutrition.

Dr Eleanor Chua Chih Yin

Dr Eleanor Chua graduated with First Class Honours in the Biomedical Sciences and Traditional Chinese Medicine double degree programme from Nanyang Technological University and Beijing University of Traditional Chinese Medicine. During her tertiary studies, she was awarded a NTU College Scholarship and the Singapore College of Traditional Chinese Medicine (SCTCM) Outstanding Student Scholarship. Ms Chua has a keen interest in skin and vascular diseases, having worked with both TCM and Western medicine skin specialists. She believes in holistic and preventive health and is a certified Yoga Alliance teacher. She has conducted several talks relating to health and diet in Beijing, China.
Dr Chua is a Singapore-registered TCM physician volunteering at Singapore Chung Hwa Medical Institution, and is currently the Vice-head of Centre of Continuing Education at SCTCM. She also teaches diagnostics of TCM and TCM disease management (dermatology and gynaecology) at the Singapore College of TCM.

Ms Fionn Chua

Ms Fionn Chua is a Senior Dietitian with over eight years of clinical experience. She graduated with a Bachelor of Nutrition and Dietetics (Hons) from Flinders University, Australia and is an accredited dietitian with Singapore Nutrition and Dietetics Association. She provides dietary counselling and nutrition support to patients in the wards and diabetes/metabolic clinic. She is part of the cardiology multidisciplinary team, where she gives dietetics input. She also runs a lipid clinic with a specialist Endocrinologist. Dr Geetha Kayambu

Dr Geetha Kayambu is a Principal Physiotherapist specialising in critical care research at the National University Hospital (NUH). She graduated with Honours in Physiotherapy and was awarded a PhD at School of Medicine, University of Queensland in 2015 from Australia. She served as Director of Research, Department of Rehabilitation, NUH from 2015-2019.
Dr Kayambu is the Physiotherapy Research Lead and is instrumental in promoting research interests amongst physiotherapists in NUH, overseeing direction of potentially high-impact research and motivating local and overseas research collaborations. She has authored and peer reviewed several publications and spoke at several national, international conferences and webinars.
Dr Kayambu has been a cardiothoracic physiotherapist for 20 years and has mentored staff and students clinically and in research. Her research interests include novel rehabilitation in intensive care, managing sarcopenia and frailty in the critically ill, skeletal and lung point of care ultrasound in intensive care and early mobilisation of critical care patients. She is a stakeholder in the Enhanced Recovery After Cardiac Surgery Programme in National University Heart Centre, Singapore.

Adj Assoc Prof Ramanathan Kollengode

Adj Assoc Prof Ramanathan Kollengode is an Adult Cardiac Intensivist at the Cardiothoracic Intensive Care Unit (ICU) at the National University Heart Centre, Singapore. He had his speciality training in Cardiac Anaesthesiology from India and is trained in Intensive Care Medicine from Brisbane, Australia. He is also trained in the management of critically ill cardiac patients and has expertise in the management of patients with mechanical cardiac support in ICU.
He is currently the Programme Director for the ICU Fellowship as well as the Advanced Fellowship Training Programmes, that has trained intensivists from around the world – some of whom are currently leading intensive care units in various countries.
Adj Assoc Prof Ramanathan Kollengode has been instrumental in establishing simulation-based courses and training programmes for medical specialists, residents and undergraduates in the region, educating them on various aspects of intensive care medicine and extracorporeal life support. He also teaches at the Yong Loo Lin School of Medicine, National University of Singapore.

In 2016, he spent a year-long clinical fellowship at the Baker IDI Institute in Melbourne, Australia with a focus on clinical obesity research and management. He is currently the Director of Obesity Centre at SGH.

Dr Lim Su Lin

Dr Lim Su Lin is the Chief Dietitian and Head of Therapeutics (Allied Health & Pharmacy) at the National University Hospital. Over her 29 years as a clinical dietitian, Dr Lim is renowned for developing many innovative tools which have been published internationally, and subsequently incorporated into mainstream practice.
Her latest innovation is an effective weight management strategy which combines a new Healthy Keto Diet, mobile health, online health coaching and behavioural and lifestyle management, that has helped many who are overweight or obese achieved significant results. She worked with food industry partners to develop Healthy Keto local favourites and nutritionally balanced meals (HealthFull) for effective and rapid weight loss.
Dr Lim has received numerous awards on her work in research, innovation and quality improvement. She was a recipient of the QUT’s Outstanding Doctoral Thesis and Outstanding Alumni Awards. She is also two-time winner of the prestigious Singapore National Day Awards – the Efficiency Medal in 2007 and the Commendation Medal in 2020.

Dr Paul Lim

Dr Paul Lim is a consultant cardiologist and electrophysiologist with the Heart Specialist Clinic at Mount Elizabeth and Parkway East hospitals and medical centres, and a visiting consultant at the Department of Cardiology at the National Heart Centre Singapore. He is also an adjunct assistant professor at the Duke-NUS medical school Singapore.
He graduated with a Bachelor of Medicine and Surgery from the National University of Singapore (NUS) in 2004 and subsequently obtained his Membership of the Royal College of Physicians (United Kingdom) and a Master of Medicine (Internal Medicine) from the NUS in 2009. He was accorded the Certificate Board Accreditation in Cardiology in 2014 and completed his fellowship at the Barts Heart Centre NHS Trust London, United Kingdom in 2017, where he underwent advanced cardiac electrophysiology training. He attained European Heart Rhythm Association Certified Electrophysiology Specialist and Certified Device Specialist Certification in 2019.

Dr Lim’s clinical interests include sports and musculoskeletal injuries, pre-participation screening, sports safety, exercise prescription and physical activity for chronic disease management. She is the public outreach director of Exercise is Medicine Singapore, a global initiative to incorporate physical activity in the prevention and management of chronic medical conditions. Dr Low graduated with a Bachelor of Medicine and Bachelor of Surgery degree at the University of Singapore and is a Fellow of the Royal Australasian College of Physicians, a Fellow of the Academy of Medicine and the College of Cardiology, Singapore and a Fellow of the American College of Cardiology.
He is a Consultant Cardiologist at the Mount Elizabeth Medical Centre, and a Visiting Consultant to the Singapore General Hospital and the National Heart Centre Singapore.
Dr Low is a founding member of the Singapore Heart Foundation (SHF), where he served as Chairman for 16 years until 2008. He is currently SHF’s Chairman Emeritus. He was also formerly the President of the Singapore Medical Association, Singapore Cardiac Society and Asian Pacific Society of Hypertension. He was also the founding President of the Singapore Hypertension Society, Asia Pacific Heart Network and a continental representative to the Executive Board of the World Heart Federation.
Dr Low has served on numerous governmental and public committees and medical advisory boards. He has also participated in the organisation of and spoken at many local and international medical meetings. Additionally, he has authored or co-authored more than 90 scientific papers and has been on the editorial board of several scientific journals.
He was conferred the Public Service Star (BBM) in the National Day Awards 2001 by the President of Singapore.

Mr Muhammad Isa Bin Mohd Musa

Mr Muhammad Isa Bin Mohd Musa has been working as a Physiotherapist at the National Heart Centre Singapore for the past 8 years. He has worked with patients with various acute cardiac conditions as well as those who come for outpatient rehabilitation. In addition, he is well experienced in working with a special cardiac population that includes Left Ventricular Assistive Device (LVAD) users and heart transplant patients. In 2021, Mr Muhammad Isa Bin Mohd Musa led a quality improvement (QI) project – Enhanced Recovery After Thoracic Surgery. This QI project focused on early mobilisation on post-operative day 0 right after the patients are done with their surgery.
His career goal is to develop a set of physiotherapy related value-based outcome measures for patients with coronary artery disease in Singapore in the future.

Ms Clarice Ng

Clarice Ng is a Senior Medical Social Worker (MSW) with 14 years of experience in organisations such as National Heart Centre Singapore, Home Nursing Foundation and Changi General Hospital. She has extensive experience working with cardiac patients and her devotion to cardiac work won her the National Day Efficiency Medal. She believes in continuous improvement and dedicates her time to organisational improvement projects. She also enjoys educating the next generation of MSWs.
Clarice completed a Masters of Public Health in 2012 in a bit to create change at the systems level. She believes system changes will create greater societal impact. To arm herself with greater knowledge and skills, she also took a Certificate in Foundational Systemic Thinking and Approaches and Satir Transformational Systemic Therapy. As an extension of her work as a cardiac MSW, Clarice is also a strong advocate for Advance Care Planning.

Mr Qamaruzaman Syed Gani

Mr Qamaruzaman Syed Gani is a Senior Physiotherapist at the National University Hospital Department of Rehabilitation, under the Cardiothoracic and Vascular Surgery team. He graduated from Nanyang Polytechnic in 2011 and was awarded the Academic Medicine Development Award (AMDA) sponsorship in 2015 to take his MSc in Cardiovascular Health and Rehabilitation. He is also passionate about interprofessional education and was the Lead Clinical Coordinator for Physiotherapy in NUH until May 2023. In addition, he is an Adjunct Lecturer at the Singapore Institute of Technology, specialising in Cardiac Rehabilitation. His areas of interest are cardiac rehabilitation, including heart failure, critical care rehabilitation. Assoc Prof Verena Tan is an experienced dietitian with diverse experience in clinical nutrition, research and academia. She is currently the Associate Professor and the Programme Leader for the Bachelors of Science (Honours) in Dietetics and Nutrition at the Singapore Institute of Technology. She is the recipient of multiple research grants working on novel nutrition solutions for the Asian population. Her research interests lie in dietary prevention and management of cardiovascular disease, cancer, sarcopenia and elderly nutrition.

Dr Victor Tan is an Associate Consultant with the Singapore Sport & Exercise Medicine Centre in Changi General Hospital. He is the first dual accredited specialist in both Internal Medicine and Sport & Exercise Medicine in Singapore. His interest lies in exercise medicine for chronic disease patients as well as population health. He is a medical officer for Team Singapore athletes for the past three editions of South-East Asia Games and Singapore Formula 1 Grand Prix. Clinical Associate Professor Tan Swee Yaw is a Senior Consultant with the Department of Cardiology and the Director of Cardiovascular Rehabilitation and Preventive Cardiology at the National Heart Centre Singapore. His research interest lies in exercise physiology, sports cardiology and preventive cardiology. He has published peer review journals in these topics. Clin Assoc Prof Tan additionally reports on cardiac CT angiography scans and is equally well-published in this field.

With a special interest for education, she is also a clinical educator for the Dietetics department mentoring and grooming the next generation of dietitians. She is a member of the Dietitians Association of Australia.

Assoc Prof Dr Ti Lian Kah

Dr Ti Lian Kah is the Director of Cardiac Anaesthesia and Senior Consultant at the Department of Anaesthesia, National University Hospital, Singapore. He is also an Associate Professor with the National University of Singapore (NUS).
Dr Ti obtained his MBBS and Master of Medicine (Anaesthesia) from the NUS and completed his fellowship in Cardiothoracic Anaesthesia at Duke University, USA. His research interests include aging and aging-related outcomes after anaesthesia and surgery, particularly the complications of delirium, acute kidney injury and atrial fibrillation. In his clinical practice, he is a keen adopter of new technologies and practices, including minimally invasive cardiac surgery, 3D echocardiography and more.
Dr Ti is currently Vice-Chair of the Asian-Australasian Regional Section of the World Federation of Anaesthesiologists (WFSA) and a Committee Member of the Scientific Committee of the WFSA. He is also the Co-Scientific Chair for the 18th World Congress of Anaesthesiologists, and was President of the College of Anaesthesiologists Singapore and the Singapore Society of Anaesthesiologists.

Ms Cherie Tong Chung Yan

Ms Cherie Tong has been a Clinical Dietitian for over 20 years with interests in malnutrition, sarcopenia, advanced nutrition support and chronic disease management. She strives to advance nutritional care through evidence-based practice, research and education. Ms Tong nurtures future dietitians as Adj Prof at the Singapore Institution of Technology, and has also guided dietitians in quality improvement projects and clinical studies. Ms Tong has pioneered and contributed to various new models of care, multi-departmental collaborations and community engagement. She has also led various projects that harnesses creativity and leverages on technology sustainably.
She enjoys sharing her experiences with other healthcare professionals and has led multiple interprofessional educational programmes and created virtual nutrition training programmes for all healthcare professionals. Ms Tong was recently appointed the Dietetic Lead under the SingHealth Regional Health System to lead and coordinate initiatives for Healthier SG. She also contributes her expertise as a member in the Diet panel under the Ministry of Health.

Asst Prof Wong Su Ren is the Principal Occupational Therapist and Research Director at the Department of Rehabilitation, National University Hospital and Assistant Professor at the Health & Social Sciences Cluster, Singapore Institute of Technology. Dr Wong works with diverse patients who require lifestyle management. As a certified life coach, Dr Wong has incorporated holistic coaching approaches into patient care to support lifestyle change.
Asst Prof Wong obtained her PhD in Rehabilitation Sciences from the University of Illinois at Chicago on Therapeutic Use of Self, which focused on the quality of client-therapist relationships to impact patient’s participation in therapy. Her research interests include improving the quality of life and quality of care for patients with complex and chronic conditions.

Adj Assoc Prof Raymond Wong

Adj A/Prof Raymond Wong is currently a Senior Consultant in the Department of Cardiology, National University Heart Centre, Singapore. He also holds the appointment of Director of Clinical Services, Department of Cardiology, and Clinical Director of Nuclear Cardiology.
Adj A/Prof Raymond Wong graduated from the Yong Loo Lin School of Medicine, National University of Singapore in 1997. He obtained the Membership of the Royal Colleges of Physicians (United Kingdom) in 2002, and completed the Cardiology Advance Specialist training at the Cardiac Department of National University Hospital in 2006. Adj A/Prof Wong went on to the Cleveland Clinic, Ohio for advance fellowship in Heart Failure Transplantation, as well as Cardiac Imaging (Echocardiography and Nuclear Cardiology) from 2007 to 2008.
Adj A/Prof Wong has special research interests in the fields of heart failure and cardiomyopathy. In addition, he is involved in many quality improvement projects within the hospital.

Assistant Professor Yeo Tee Joo is a Senior Consultant with the Department of Cardiology at the National University Heart Centre, Singapore and heads the Cardiovascular Prevention and Rehabilitation unit. He trained in Cardiovascular Prevention and Rehabilitation at the Toronto Rehabilitation Institute from 2014 to 2015, and Sports Cardiology at St George’s University of London from 2015 to 2016.
Asst Prof Yeo is a World Health Organization Ischaemic Heart Disease Rehabilitation 2030 Development Group Member, the Singapore representative for the International Council for Cardiovascular Prevention and Rehabilitation and a Board Member at the Singapore Heart Foundation.

Ms Yeo Wee Ting

A physiotherapist for the past eight years, Wee Ting has been helping patients with various cardiac conditions. She specialises in critical care for both heart and lungs surgery by providing early mobilisation and acute rehabilitation immediately post-operation to optimise their physical and cardiopulmonary function. She also manages patients with heart failure by helping them understand their conditions and improving their quality of life through individualised patient education and phase 1 to 3 cardiac rehabilitation.

Ms Natalie Yeo

Ms Natalie Yeo is a Senior Dietitian at the Singapore Heart Foundation (SHF).
She graduated with a Bachelor of Nutrition and Dietetics from Queensland University of Technology, Brisbane and has worked at various units in Khoo Teck Puat Hospital (KTPH) including the Cardiac Inpatient Unit. At KTPH, she conducted a programme to empower post-discharge heart patients with nutritional knowledge to help them make sustainable lifestyle choices. Ms Yeo also supported the Foodservices Unit during the COVID-19 pandemic by developing an emergency menu to ensure patients were served nutritionally balanced meals.
At SHF, Natalie continues to help her clients and the greater community experience the difference of a heart-healthy diet through educational workshops and one-to-one consultations. She believes that everybody is unique and a tailored and holistic management is essential to cultivate a good relationship with food.

Dr Yong Quek Wei

Dr Yong Quek Wei graduated from the National University of Singapore in 1987. He completed general cardiology training at Tan Tock Seng and National University Hospital, followed by advanced training in Echocardiography at St Vincent’s Hospital, and The Victorian Heart Institute at The Epworth Hospital, Australia in 1999. He received further training in Cardiac CT at the German Heart Centre (Deutsches Herzzentrum) in Munich under the Ministry of Health (MOH)’s HMDP scholarship.
Dr Yong was the past Vice Chairman of The Chapter of Cardiologists, College of Physicians, Academy of Medicine, Singapore and sits on the Panel of the Specialist Training Committee under the Specialist Accreditation Board.
He is a Fellow of the Academy of Medicine (Cardiology) and of the Physicians of Ireland. He is also a Board and Committee Member of various national bodies, namely The National Resuscitation Council, The National Medical Research Council Peer Review Subcommittee for Cardiovascular/Respiratory Research, and a Board Member of various taskforce committees for MOH, Singapore. He specialises in the field of lipidology, insulin and cholesterol metabolism, insulin resistance, hypertension, heart failure, cardiac epidemiology, vitamins and homocysteine, echocardiography and cardiac CT.
